What is the PACE Center?

The PACE Center is an interprofessional health and wellness center that focuses on meeting the health promotion and disease prevention needs of older adults. 

What is included in a PACE Center visit? 

At your visit, our team will provide no-cost health screenings and a personalized list of recommendations and health information based on your needs and circumstances. 

What types of health screenings are offered? 

  • Blood pressure
  • Cholesterol
  • HbA1c for diabetes 
  • Hearing
  • Vision
  • Falls risk and safety issues
  • Stress
  • Sleep apnea
  • Social isolation
  • Caregiver support
  • Vaccination status 
  • Oral health 

When are appointments available? 

Telehealth appointments are available each Wednesday between 8:30 AM and 1:00 PM between the months of May to August.
