CPC1    Clinical description: This 75 year old female presents with a history of an upper lip swelling present "for a long time". What brings her to you is that she recently began experiencing paresthesia of her upper lip in that area.
CPC2   Clinical description: This 65 years old male, recent immigrant, presents for evaluation of an obvious facial asymmetry. He recalls that it has been present for years.
CPC3   Clinical description: This patient returns to clinic for follow up exam, several weeks after extraction of multiple non-maintainable, mobile, and carious teeth in the right mandible with "periapical pathology". He reports no pain but an enlargement of the soft tissue in the area of extractions.
CPC4   Clinical description: A 14 year old male presents with a complaint of an increasing painless swelling of his palate. The swelling is bony hard on palpation.
CPC5   Clinical description: This renal transplant patient presents to your office for comprehensive dental care. He takes cyclosporine and nifedipine.
CPC6   Clinical description: This 20-year-old healthy male presents for comprehensive care with a recent history of shifting of his maxillary left anterior teeth over the past two months. Clinical periodontal examination reveals no periodontal disease. Radiographs are taken.
CPC7   Clinical description: This 29 year old patient presents to your office for the first time for comprehensive care. You note an unremarkable clinical exam but find a radiolucency on periapical radiograph.
CPC8   Clinical description: This lesion is noted in an 11 years old boy, born with this mass on dorsal surface of tongue. No clinical symptoms. No, if any, change in size.
CPC9   Clinical description: This 40 year old female presents for evaluation of an intraoral swelling that has been present for some time but has recently increased in size. Upon clinical examination, you also note a swelling in the neck in the submental region. Both are compressible.
CPC10   Clinical description: This healthy 23 years old male dental student presents with a complaint of a non painful palatal swelling which he has noted for several months. It occasionally increases and decreases in size. The area is soft on palpation. The patient has also a periapical radiolucency associated with teh apex of tooth #12.
CPC11   Clinical description: This 53 year old male presents with generalized enlargement of gingiva, focal palatal petechiae, and a three week history of extreme fatigue. Patient subsequently noted eruption of multiple cutaneous nodules of the face, neck, scalp, and chest. Diagnostic tests included: H&E, immunoperoxidase and histochemical stains, peripheral blood studies, flow cytometric analysis, and bone marrow biopsy.
CPC12   Clinical description: This 55 year old male presents for evaluation of his upper denture which was made approximately 30 years ago. Upon clinical examination, you find the denture to be ill fitting and notice what is seen in clinical photo. You adjust his denture. Three days later he returns with findings on second photo. You perform a biopsy.
CPC13   Clinical description: This patient is a 46 year old male who presents to your office complaining of a mass of the upper lip slowly increasing in size . It has become bothersome to him, and he has traumatized the area on multiple occasions. Clinical examination reveals another similar but smaller swelling on the posterior buccal mucosa.
CPC14   Clinical description: This 40 year old male presents with an unremarkable medical history but is concerned about a firm, painless swelling in his left mandible which is preventing him from wearing his removable partial denture. Clinical examination reveals an enlarged area in the left mandible with normal overlying mucosa.
CPC15   Clinical description: This 14 year old patient presents to the orthodontist for "braces" because of "tooth crowding". A panoramic radiograph is taken which reveals the provided radiograph. Similar radiographic findings were noted in the right mandible. There is vestibular tenderness but no real swelling. Clinical examination reveals multiple facial "freckles" similar to his father.
CPC16   Clinical description: This 55 year old male presents for evaluation of a swelling in his right neck, which he reports has been slowly growing over several months. His medical history history is unremarkable except for a 30 pack year history of cigarette smoking. Upon palpation the swelling is not tender and doughy.
CPC17   Clinical description: A 23 year old white female presented to her dentist with a firm palatal mass which she noticed a year ago. She claims that the bump has been growing slowly over this time period and is painless.
CPC18   Clinical description: A 28 year old female presented with a firm, lobulated mass of posterior hard palate and alveolar ridge. The mass was slow-growing, painless, and had been present for two years. The surface of the mass is focally ulcerated and covered by a white slough.
CPC19   Clinical description: Patient is 40 year old male who presents to you for dental evaluation. He states his teeth have recently become mobile and he has been feeling very tired.
CPC20   Clinical description: This patient is a 9 year old male who presents to your office for routine exam with a complaint of loose teeth.
CPC21   Clinical description: This patient presents for recurrent non painful swelling of his left mandibular vestibule.
CPC22   Clinical description: An 18 year old female patient presents with a small, sessile mass of the left mandibular mucobuccal fold which has been growing slowly for just over a year. Tilting and displacement of the premolars in this region are seen.
CPC23   Clinical description: Your routine dental films indicated multiple radiolucences and some impacted teeth. His mother states his father had a lot of cysts removed as well.
CPC24   Clinical description: This 60 year old Turkish male complains of multifocal painful oral lesions. He also has "sores" on his skin, genital mucosa and conjunctivitis. He says oral lesions began with a "canker sore" 3 month ago.
CPC25   Clinical description: This patient presents for retreatment of endodontically treated #31 you note a radiolucency in the alveolar bone anterior to #31
CPC26   Clinical description: This elderly female presents to you complaining of increasing dry mouth, and difficulty swallowing food. On clinical exam you note bilateral preauricular swelling.
CPC27   Clinical description: This HIV positive patient has recently been complaining of intraoral mucosal soreness.
CPC28   Clinical description: This 50 year old male complains he can no longer wear his lower denture because of an intraoral swelling.
CPC29   Clinical description: This 24 year old healthy female has been routinely followed by her dentist for years. She has recently developed a radiolucency in the anterior mandible.
CPC30   Clinical description: These two siblings present to your office for dental evaluation for the first time. Their mother is concerned that their teeth are not erupting and the younger child has an increasing facial swelling.
CPC31   Clinical description: This patient presents with a chief complaint of numbness of his lower lip present for a few weeks. Intraorally there is a soft tissue swelling of the left retromolar pad area.
CPC32   Clinical description: This 60 year old female presents to your office for recent onset of tooth pain. You note some bony swelling on your oral exam but no active infection aside from multiple carious teeth.
CPC33   Clinical description: This teenage girl presents to your office complaining of facial asymmetry. There is no pain and the patient says this has developed over a prolonged time. She also has pigmented areas of her skin. Her mother states that she entered puberty at 10 years of age.
CPC34   Clinical description:This patient has a tender swelling of his palate of 2 weeks duration. On exam you note a facial asymmetry.
CPC35   Clinical description: A 22 year old female presented to her dentist with a focal swelling of the mandible in an extraction site just posterior to her second premolar. The swelling was soft, sessile and pink in color. The patient was not aware how long the lesion was present. Recently, the patient was being worked up for persistent abdominal pains.
CPC36   Clinical description: This 10 year old presents to you for increasing mobility of teeth and gingival swelling. Patient is also visiting an endocrinologist as she has recently been diagnosed with diabetes mellitus.
CPC37   Clinical description: This patient has been wearing this denture for 25 years. Your clinical exam is unremarkable except for this palatal lesion. The patient was unaware of it.
CPC38   Clinical description: This 55 year old male presents with a complaint of right mandibular swelling. He had a "cyst" removed 2 years ago in that area when his dentist extracted a molar tooth. The dentist did not submit the tissue for histopathology.
CPC39   Clinical description: This 80 year old male has an non healing "sore" of his lower lip for 8 months.
CPC40   Clinical description: This middle aged female presents to you for evaluation on "periodontal disease." Despite numerous dental cleanings her gingiva are tender and continue to bleed and enlarge.
CPC41   Clinical description: This 58 year old male has multifocal mucosal "sores" present for at least 3 months. They are tender most of the time but some days are worse than others. Some areas spontaneously resolve where as others have been present for months.
CPC42   Clinical description: This asymptomatic patient presents to your office for routine recall examination. You note he has had some shifting of mandibular anterior teeth. There are no bony changes noted on periapical films taken 2 years ago.
CPC43   Clinical description: This child is brought to your office by his mother who is concerned about a recent increase in mobility of his teeth.
CPC44   Clinical description: Patient has a slowly enlarging right mandiblular mass and mobility of teeth.
CPC45   Clinical description: This 70 year old female with a history of breast cancer presents complaining of numbness of her lower lip.
CPC46   Clinical description: This asymptomatic palatal nodule is noted on your recal exam.
CPC47   Clinical description: You note this palatal swelling on your clinical examination. The area is asymptomatic.
CPC48   Clinical description: This patient initially presents for evaluation at 8 years of age. The patient had multiple nodules and pigmented areas of the skin. As he aged, he continued to develop more lesions with intrabony and intraoral swellings as well. All lesions are asymptomatic.
CPC49   Clinical description: This slowly enlarging lesion of the buccal mucosa was thought to be a traumatic fibroma.
CPC50   Clinical description: This middle aged male reports a history of rapidly increasing left side facial swelling with tenderness over the affected area.
CPC51   Clinical description: This patient presents to you for comprehensive care. The erythematous lesions of her face have been present since birth. She is taking tegretol for seizures.
CPC52   Clinical description: This patient presents to your office for an implant consultation. To replace missing posterior mandibular teeth.
CPC53   Clinical description: This patient presents to your office for dental evaluation. Upon thorough examination you note these lesions on his tongue. The patient has a significant history of cigarette use.
CPC54   Clinical description: This elderly male presents to your office complaining that he can no longer wear his mandibular removable partial denture. Upon examination you note macroglassia.
CPC55   Clinical description: This patient has a recent onset of difficulty swallowing and moving his tongue. He has been using smokeless tobacco for 50 years.
CPC56   Clinical description: This 75 year old male complains of this painless crusted lesion which has not resolved. It has been present for months.
CPC57   Clinical description: A patient of record in your office brings in her 2 month old child to have you look at a swelling of the child’s maxillary alveolar ridge. You note an expansion of the buccal aspect that is firm and bluish in color.
CPC58   Clinical description: During this 66 year old male patient’s 6 month recall you note a 1 cm. soft, yellow nodule of the left ventral tongue/floor of mouth. He has been unaware of the lesion.
CPC59   Clinical description: This 28 year old male presents to your office with a slowly enlarging, painless submental swelling. He reports that it has been present for at least one year.
CPC60   Clinical description: This 57 year old white female has the chief complaint of painful lesions on her lips, tongue and cheeks.
CPC61   Clinical description: The mother of this 13 year old boy reports that these lesions “just popped up” yesterday. The patient has a sore throat and his temperature is 100°F. In addition to the lesions on his palms, he has similar lesions on the soles of his feet. What is the etiology of this condition?
CPC62   Clinical description: This 17 year old male presents with the chief complaint of a slowly expanding swelling of the left side of his face. He first noticed the swelling about 2 years ago.
CPC63   Clinical description: This 9 year old female patient has a history of painless maxillary expansion for the past 5-6 years. She and her parents are interested in orthognathic surgery to correct the cosmetic defect. Her medical history is significant for a microcytic, hypochromic manemia. She has periodic blood transfusions to treat the anemia. She also has hepatosplenomegaly. Lateral skull radiographs show a “hair-on-end” appearance of the calvaria.
CPC64   Clinical description: Your HIV positive patient comes to the office complaining of fever, sore throat and non-healing ulcers of the oral mucosa. Biopsy of the ulcerations reveals these findings.
CPC65   Clinical description: This 58 year old female presents to your office for comprehensive care. She has been having pain in her legs, back and jaws. She has been to 2 other dentists who were unable to stop the pain. You noticed when the patient entered the room that she had bowed legs. Laboratory blood values reveal: alkaline phosphatase elevated calcium within normal limits phosphorus within normal limit. The skull radiograph is from a different patient with the same condition.
CPC66   Clinical description: This 26 year old male presents to your office with the chief complaint of “sore gums”. His medical history is unremarkable. The patient reports that the last few days he has been having symptoms of an upper respiratory tract infection with a runny nose, cough and a mild fever.
CPC67   Clinical description: This 42 year old black female has the chief complaint of “bumps” on her jaw. She reports no pain associated with the lesions. You take a panorex radiograph and it reveals these findings.
CPC68   Clinical description: Your 50 year old black female patient has multiple papules on the skin that give it a rough texture /and are often pruritic. You note changes in the patient’s finger nails. The oral mucosa has multiple flat topped papules that are confluent in areas giving a cobblestone appearance to the oral mucosa.
CPC69   Clinical description: This 6 year old white male comes to your office for his first dental visit. You note he has loss of some deciduous teeth with no eruption of the permanent replacement.
CPC70   Clinical description: This 51 year old white female recently moved to your area and comes in for an initial office visit. You note the numerous small red papules on her tongue, lips and buccal mucosa. The patient reports they have been present since she was a child.

Her medical history is significant for a brain abcess 7 years ago for which she was hospitalized. She also reports that she has frequent nose bleeds.

CPC71   Clinical description: Patient had extraction for cosmetic purposes.
CPC72   Clinical description: his 37 year old HIV positive male has multiple cutaneous papules of the face of approximately 3-4 weeks duration. This condition is usually seen in children causing far fewer lesions, but, as seen in this patient, can cause extensive morbidity in the immunosuppressed.
CPC73   Clinical description: This 48 year old female presents with the chief complaint of discoloration of the gingiva. She reports no significant medical history.
CPC74   Clinical description: This 98 year old male was transported from his nursing home with doctor's orders to evaluate the "sores" in his mouth and on his lips. Multiple ulcerations are noted on the tongue and oral mucosa. These findings were noted cutaneously.
CPC75   Clinical description: This patient presents to your office with a firm nodule on the dorsal tongue. The lesion was surgically removed without complication.
CPC76   Clinical description: This 10 year old was referred to you by her pediatrician for evaluation of the ulcerated lesions on her lips and in her mouth. The patient’s medical history was significant for periodic abdominal cramping and diarrhea.
CPC77   Clinical description: An 18 year old male presents with a 1.5 x 0.9 cm. firm pink mass on the buccal gingiva in the region of tooth #29. The patient reports that the lesion has been present 5-6 months. It is asymptomatic, but bleeds upon brushing.

The surgeon reported expansion of the facial cortical plate.

CPC78   Clinical description: This 2 and a half year old male presents to your office with his parents. They are concerned about the color of the patient’s teeth. The parents would like to know if they are decayed and if the permanent teeth will be affected.
CPC79   Clinical description: This 39 year old male was referred by his doctor for evaluation of the pigmented areas on the buccal mucosa. All other oral findings are within normal limits. Special stains reveal the presence of dendritic cells positive for HMB 45.
CPC80   Clinical description: his 50 year old female comes to the office to have a "bump" looked at. Intraoral exam reveals an area of ulcerated swelling between teeth # 5 and # 6. The patient reports it has been slowly increasing in size over the last 6 months.
CPC81   Clinical description: This 76 year old male has a chief complaint of recent swelling of the lower right side of his face. He reports this started about 4 months ago. His evaluation and work-up included radiographs, a CT scan, scintigraphy and a biopsy. Immunohistochemical stains show cells positive for PSA.
CPC82   Clinical description: A 36 year old female is referred to you for comprehensive care. Radiographs reveal this mixed radiolucent/radiopaque lesion of the mandible.
CPC83   Clinical description: This 24 year old male is requesting cosmetic repair of his anterior teeth. His medical history is significant only for a hospitalization for illness at two years of age. Current medications include tetracycline for acne and one multi-vitamin per day.
CPC84   Clinical description: This 13 year old female has the chief complaint of periodic swelling of her lips, especially the upper lip. This has been happening most of her life, but recently the swelling has not subsided. The patient was treated with intralesional injections of Kenalog.
CPC85   Clinical description: This 64 year old white female has been a patient of record in your office for 15 years. She presents with mucosal lesions. Your records show that she has never had these lesions at any previous visit. Her medical history is significant for calcium channel blockers (x 10 years), periodic use of antidepressants and a cholesterol lower drug (x2 months)
CPC86   Clinical description: The leukoplakic and erythroplakic lesions noted on the oral mucosa of this 60 year old black male have been present about 5 days, according to the patient. His past medical history is significant for a left below the knee amputation. He is currently taking NPH Iletin I once per day, 1 baby aspirin per day and today he took the last pill of a 2 week course of ampicillin, 250mg q.i.d., given for a sinus infection.
